The following items are NEEDED to service a case:

1. Food Stamps/AFDC Program Verification:
   1. Referral Letter
   2. Appointment Verification Form for Food Stamps/AFDC/TANF
   3. Food Stamp/TANF Action Letter
2. Residence:
   1. Rent/Lease Receipt or Statement from Landlord
   2. Mortgage Payment Book/Statement
3. Cost of Shelter:
   1. Rent/Lease Receipt or Statement from Landlord
   2. Mortgage Payment Book/Statement
4. Income (last 30 days):
   1. Last four (4) pay stubs or strike verification
   2. Current unemployment card
   3. Letter of amount of S.U.B. or letter of eligibility
   4. Social Security check or current letter of claim or ineligibility
   5. TANF/AFDC award letter or referral letter
   6. Written proof of child support (Court order)
5. Size of Family:
   1. Social Security cards for everyone in household
   2. Birth certificates or hospital records for everyone in household
   3. Marriage certificate (if applicable)
   4. Divorce papers (if applicable)
   5. Green card if not a citizen of the United States
   6. Naturalization papers (if applicable)
   7. Driver’s License or Picture I.D. for anyone 18 years or older
6. Medical Expenses:
   1. Statement from doctor for special diets
   2. Statement from doctor for temporary disability
   3. Paid receipts for prescriptions
   4. Doctor’s receipts
7. Assets:
   1. Savings account book
   2. Bonds, ERA’s, CD’s, 401-K’s, etc…
   3. Checking accounts/books
   4. Credit Union Savings
8. Expenses (last 30 days):
   1. All Credit Card payments
   2. Utility Bills (including complete telephone bill)
   3. Car Loan(s)
   4. Cable Bill
   5. Other (i.e.; student loan payments, second mortgage payments, other Loans, etc…)
